Annual Faculty Performance Review
All full-time regular faculty members must be evaluated each year. The evaluation is conducted by the department chair, division chair, or dean, depending on the administrative organization of the school or college.
The department chair, division chair, or dean, depending on the administrative organization of the school or college, will fill out the Annual Faculty Performance Evaluation Form (Opens in Box - Please download the form to your computer, and open with Adobe Acrobat), and all full-time regular faculty will fill out the Planning for the Year Ahead (opens in Box) document. Each of these documents must be used in all faculty evaluations; please note the Planning Document Instructions document gives guidance for completing the Planning for the Year Ahead document.
The forms are due to the dean on February 12 of each year.
Each school or college has established its own practices regarding when the faculty members whom the chairs are evaluating would need to submit information about their accomplishments for the year to the chairs.
